Knowing of the inherent risks, dangers, and rigors involved in the activities, I certify that my family and I, including any minor children, are fully capable of participating in the activities. I assume full responsibility for my family and myself, including any minor children, for bodily injury, death, loss of personal property, and expenses thereof. In consideration of services to be received, the undersigned, for himself and his heirs and assigns, hereby releases the concessioner and the concessioner's employees from any liability for claims or lawsuits brought by the undersigned and arising out of the activities provided by the concessioner. Under Colorado Law, an Equine Professional is not liable for an injury to or the death of a participant in equine activities resulting from the inherent risks of equine activities pursuant to Section 13-21-119, Colorado Revised Statutes. Further I understand that all trips shall be paid for in full or ensured by a credit card number, travelers check, or a personal check approved by Frying Pan Anglers or his agent (sorry, no out-of-Colorado checks accepted), and that refunds shall be made only at the discretion of Frying Pan Anglers. Trips cancelled less than two-weeks prior to requested date shall be charged to the client. ___________________________________ Transportation shall be provided by your guide, unless you request other arrangements. We will do our best to fulfill your date request and guide requests. Before I can hold any booking, We need a credit card number to guarantee the trip. DO NOT SEND YOUR CREDIT CARD NUMBER OVER THE INTERNET. We will call you during day-time hours to secure your booking. Cancellation Policy. If you want to cancel a booked trip, please contact us at least 48 hours prior to the trip. If you do not then we reserve the right to charge you $100. You must understand that once a trip is booked and a guide is allocated to you, that guide is not available for any other work. If you do not cancel in the required time but we are able to give the guide another booking we will refund you the $100.
